A family approach to healthy aging

AVIV stands for Aging with Vibrance, Independence and Vitality. AVIV fulfills JF&CS' mission to deliver aging services holistically - that is, caring not just for the older adult but considering the needs of the whole family with a comprehensive wellness plan.

It takes a plan: Geriatric Care Management

The most comprehensive approach to AVIV services is through Geriatric Care Management. AVIV offers Care Managers who are trained to conduct in-home assessments. They look at the older adult’s physical, functional, emotional and even financial situation.
Step by step, the Care Manager leads the client and caregivers through:

• In-home Assessment, with an R.N. as needed
• Development of a Wellness Plan
• Service Coordination
• Ongoing Monitoring
• Financial Planner and Legal Advisor Available
