Direct Service Professional Developmental Disabilities – Day Program F/T Provide and oversee Training and Support for all clients at the CREW Day Program, including but not limited to organization, planning, co worker relations, task and specific skills training, sensory stations, and coordination for community activities. Program provides support for social, emotional, physical and spiritual development of all clients The team lead will be responsible for written documentation of progress, case notes or Action Plans of each client. Oversight and Documentation of all Medical Administration Sheets (MAR), Goal Tracking, File Flagged Issues, Attendance and other required written work. Live-in House Manager, Independent Living Program Developmental Disabilities F/T Supervision and performance management of house staff; onsite training and orientation of new staff; evaluate and respond to emergency situations; ensure maintenance of house and consumer budgets; assist in all purchases; assist in development of Individual Service Plans (ISP) and Service Plan of Care; teach and monitor consumers in all areas of independent living as per ISP and Service Plan of Care; provide training and support in all daily living skills; provide transportation and accompany consumers to appointments, outings, etc. Required Education/Experience: Bachelor’s or associates degree preferred, preferably in area of human services.
