Mission of Service: To provide supports to adults with developmental disabilities living
in the community that enables and/or enhances self-sufficiency
Description of Program: The Zimmerman-Horowitz Independent Living Program (ZH-ILP) is
a highly individualized residential program that enables people with developmental disabilities
to choose creative living options. The program offers a range of support from a few hours a
week of case management or consultation, up to full time direct care support in the clients
home 24/7. The ZH-ILP focuses on supporting each person and their individual needs. Areas of
support include:
- Money Management (banking; paying bills; balancing checkbooks)
- Housekeeping (facilitating a routine for cleaning)
- Laundry
- Cooking
- Grocery Shopping
- Nutrition
- Recreation
- Transportation
The ZH-ILP serves adults with developmental disabilities, 21 and older. Participants sign
a contractual agreement stating their willingness to work with program staff toward
individual goals. ZH-ILP staff work collaboratively with participants & their families to
identify an appropriate living situation, roommates & staffing profile.
