Private Home Care Services, which provide medical and non-medical care wherever a person lives will now be provided exclusively by JHLC through The One Group. Care Coordination Services, also known as Geriatric Care Management Services, help older adults and their caregivers determine what their specific needs are and what services are available to meet those needs. These services, in addition to Geriatric Clinical Services which include older adult individual and group counseling services, will be provided by JF&CS.
Current cooperative efforts between the two organizations include JF&CS social work support for the Weinstein Hospice Palliative Care program and for the monthly caregiver support groups held at The Cohen Home and Berman Commons. JHLC’s The One Group provides support for many Holocaust Survivors enrolled at JF&CS, and Weinstein Hospice staff has conducted in-service training for JF&CS social workers on hospice and palliative care.

Throughout the transition process, great care has been taken by both organizations to assure continuity and consistency for clients and staff impacted by the change. The conversion should be complete in early 2016.
